Please prepare for your visit to The Hearing Center of Jackson by bringing:
  • Health Insurance
  • Card/Information
  • Physician Referral
  • A friend or family member to take notes/help with questions
  • A list of your questions/concerns
  • Bring previous audiogram or
  • VNG test results when appropriate
  • Completed Patient Forms

Billing & Insurance

Please call the toll-free number on your insurance card for a description of your benefits.

Audiology benefits depend on your individual insurance plan. We may be participating or allied providers; however that does not guarantee payment. Please contact your insurance carrier to confirm that audiology services are included on your plan before your office visit. In order for Medicare to cover audiological testing you MUST be referred by a physician and we must have that written referral on file. If you have Medicare, please obtain this referral from your physician or have it faxed to our office prior to your appointment.

Other insurances may require a physician referral; please check with your insurance to determine if this is required. If you have questions regarding insurance and/or payment options, please contact our office.
